Note: All player registration forms are sent to a soccer club, not MAYSA. Contact information for all MAYSA soccer clubs

• MAYSA Liability Waiver & Medical Release (only) with Concussion Infomation
• MAYSA Player Transfer Form
  (for Recreational players requesting a transfer to a different club)
• Medical Treatment Statement/Release of Liability Form (For Parent/Guardian to fill out, for under 18 player. This form does not contain Concussion Information. Please use complete MAYSA registration/waiver form above instead if you have not yet seen the concussion information.)

WYSA Forms

  WYSA Competitive Player Transfer form (required if transferring to a different club)

  USSF/WYSA Permission to Travel Form (required for Wisconsin teams traveling out of state)
