Release of Liability and Consent for Medical Treatment form
- * CONTINUING THIS YEAR * Every team (including Madison area teams) must present a completed waiver signed by a parent or guardian for each player participating in this event. These documents will be checked by tournament staff at team check in and must be kept on the sideline of every game during this event.
- Out of state teams MUST use this waiver. Out of state registration forms, waiver, or medical release statements will not be honored.
- Wisconsin teams may present a copy of their player registration form provided that it includes language releasing WYSA and/or MAYSA in the release of liability and medical waiver text.
Guest players are those from teams in other clubs. Up to 3 guest players are permitted per team for this event.
Club Pass players, rec or classic players from different teams in the same club, are considered part of the team's formal roster and are not counted against the 3 guest player limit. A team may use up to 4 club pass players. (This does not apply to state level players, who must be used as 'guest' players.)

There will be a fee to park in all Reddan Soccer Park parking
lots. Parking passes cost $10/weekend ($6/day) at the event. Parking passes may be purchased in advance for $8/weekend. Advance Discount Parking Order form. Please mail form and fee by the deadline to the address on the form.
