Player Waiver (view)

• * CONTINUING THIS YEAR * Every team (including Madison area teams) must present a completed waiver signed by a parent or guardian for each player participating in this event. These documents will be checked by tournament staff at team check in and must be kept on the sideline of every game during this event.
• Out of state teams MUST use this waiver. Out of state registration forms, waiver, or medical release statements will not be honored.
• Wisconsin teams may present a copy of their player registration form provided that it includes language releasing WYSA and/or MAYSA in the release of liability and medical waiver text.

This document may be required by your state association in order for them to grant permission to travel out of your home state. Please check with your State Association to inquire as to what exactly they require.
