Teams traveling from outside of Wisconsin:
Release of Liability and Consent for Medical Treatment Form
- Every team must present this form, signed by a parent or guardian, for each player participating in this event. These documents will be checked by tournament staff at team check in and must be kept on the sideline of every game during this event.
- Out of state teams MUST use this waiver. Out of state registration forms, waiver, or medical release statements will not be honored.

This document may be required by your state association in order for them to grant permission to travel out of your home state. Please check with your State Association to inquire as to what exactly they require.

If your team is traveling to Wisconsin from out of state, please begin the process of acquiring permission to
travel from your State Association. In some cases, it can take more than 30 days to acquire.
